Addressing walk-in client needs
Percentage of time over term member will spend with this program:
Member will meet with clients who walk in to the NSC office with health-related needs.
Member will triage the urgency of the need, identify next steps and make a plan with the client for meeting the need.
Member will refer the client to the appropriate Health and Wellness team member.
Member will meet with approximately 25 walk-in clients weekly
